GENERAL AFFIDAVIT
Passport or Valid I.D copy required

• I understand that by availing the service of Visit-Philippines, Brand of Eliza les Ailes du Voyage, BALIKBAYAN Services & Cargo I am paying a consolidated rate or sending my package together with other shippers;

• I understand that if I send prohibited, restricted items or items of commercial quantity intended for sale in the Philippines, there is the risk that of Visit-Philippines, Brand of Eliza les Ailes du Voyage might be delisted by the Bureau of Customs of the Philippines, in the official roster of accredited forwarders recognized by the Bureau of Customs of the Philippines;

• If a case is filed by the Bureau of Customs against the accredited forwarder (Associated Freight Consolidators Inc or AFCI) I, or my consignee, undertakes to pay for ail the cost of the litigation or the case including lawyers fees and other incidental costs;

• I also undertake to provide a copy of my passport, a detailed list of the items included in my package and if my package includes luxury items or electronic gadgets, I undertake to immediately provide the Bureau of Customs of the Philippines, my receipt or sales invoice for purposes of computing the luxury taxes or duties, and undertake to pay them;

• I understand that, by providing the name of the consignee as well as its address, he or she will automatically be my representative (in case I am not available) in any hearing, case, negotiation or meeting with the Bureau of Customs of the Philippines ; and that the consignee's address I have provided shall be the official address for sending subpoena in case needed by the Bureau of Customs;

• In case of issuance of warrant of seizure by Bureau of Customs of the Philippines I will reliquish ownership of my package and not hold Visit-Philippines, Brand of Eliza Les Ailes du Voyage or AFCI accountable thereof.

• Should the Bureau of Customs of the Philippines require any other documents from in relation to my package, I or my consignee undertakes to immediately cooperate with them and provide such other documents.

Unaccompanied baggage
(Security Declaration & Consent)

• All goods were personally packed by me, and have been in my constant possession since packing.

• I have declared all electrical items on the packing list and have removed their power sources.

• I have read the dangerous goods and restricted items list and confirm none are packed in these shipments.

• I accept whilst every effort will be made to achieve delivery times, these are not guaranteed.

• I understand that the Eliza service fee does not include any additional charges such as, custom fees, storage fees, quarantine fees, import duties, VAT or any unforeseen fees and that if these additional fees occur I will be responsible to pay.

• I am aware that any false statement may render me liable to prosecution.

• The Eliza service fee does not include any additional charges such as: custom fees, storage fees, quarantine fees, import duties, VAT or any unforeseen fees. The Eliza service fee is limited to PARIS to Airport or PARIS to Door. Any of the additional charges will be billed to the shipper. SPECIALLY SIARGAO ISLAND

• I confirm that the fragile and breakable items that are part of my persona! effects have not been professionally packed by a recognized and approved Household Removal packer and that I do not herby, for myself, my heirs, executors and assigns, indemnify and hold harmless Eliza and/or its appointed Agents against ail and any claims, loss and/or damage which may result during transit of my shipment to its final destination.

• I further acknowledge that one of RPX Ltd terms and conditions of carriage is that all goods of a fragile nature must be professionally packed by a recognized and approved packer of household goods and persona! effects.

• I acknowledge that Eliza and its Agents will only accept responsibility for damage or loss of the fragile goods that are professionally packed by a recognized and approved packer of household goods.

• I acknowledge that it is on the basis of this indemnity that I agree that my persona! effects can be transported by Eliza and its Agents without certificate of invoice to verify that my goods have been professionally packed.

• For safety/security reasons the following items are labelled as "Dangerous Goods" and MUST NOT be packed with your unaccompanied baggage and/or persona! effects:

• Compressed gasses (flammable, nonflammable or poisonous)

• Oxidizing substances (i.e.) bleaching powders & peroxides

• Corrosive materials (i.e.) mercury, acids & batteries

• Industrial products (i.e.) solvents containing chemicalswhich can cause fumes of corrosion

• Explosives

• Radio active materials

• Fresh Food

• Medicines

• Currency/Coin

• Jewels, Precious articles
